Rail Accident Investigation Branch (RAIB) has today released its report into accidents involving a wheelchair rolling onto the track at Southend Central, 28 August 2013; and a pushchair rolling onto the track at Whyteleafe, 18 September 2013.

RAIB has made four recommendations and also identified one learning point.

Rail Accident Investigation Branch (RAIB) has released its report into a fatal accident at Barratt’s Lane No.2 footpath crossing, Attenborough, Nottingham, 26 October 2013. RAIB has identified one learning point related to the design of gates at crossings of this type. It has made no recommendations.

Rail Accident Investigation Branch (RAIB) has released its report into a passenger becoming trapped in a train door and dragged a short distance at Newcastle Central station 5 June 2013.

RAIB has made six recommendations.

The way the RAIB phrased the report summary etc it was as though it was a nuclear train and they were trying to hush it up, although the photos proved otherwise. Seemed reluctant to mention the type of container it was in the summary, in a rather suspicious way. Certainly I was expecting it to be one until I saw the first relevant image of the wagon. Although are nukes permitted to travel at 70mph?

RAIB report into penetration of a tunnel between Old Street & Essex Road (KX Sim), 08-Mar-2013: no injuries fortunately.

And in Toronto about 6 weeks ago.

I gave up trying to get to the office that day after it took over half an hour to go 1 stop, less than a kilometre, on the other side of town.

Thankfully, no-one was injured in either incident.

Thankfully this one wasn't nearly as bad as it could have been if the train had struck the van! I don't think I have heard of a closer near-miss than 100mm!

Essentially "leaves", resulting in train sliding the entire way into the platform, but they also point out that Voyagers seem to be especially bad when it comes to handling low adhesion compared to other passenger units, and suggest the fitting of automatic sanders (rather than the current "emergency" sanders).
