"Main line" is the restriction that's bothering me, as it's somewhat in the eye of the beholder.

Two candidates are Zürich HB, but that depends on whether you consider the S-bahn tunnel to ZH-Stadelhofen as "main line", and Stuttgart Hbf, but that also depends on how you treat the S-bahn tunnel, and on how far advanced Stuttgart 21 now is.

Or maybe it's something else, such as Madrid Atocha or Chamartin, or, better still, Barcelona Sants. All appear possible.
I take Main Line to part of the national rail network so trams/metros are excluded. S-Bahn are included unless they are a physically isolated network (no shared tracks) eg Hamburg/Berlin. I would exclude the SZU at Zurich, but include the S-Bahn.

Zurich has three sets of platforms at a "-1" level , but I don't know if they are all the same level or not.
Madrid Atocha has two levels, one for long distance trains and a lower level for the suburban Cercanias lines.
Madrid Chamatin is all on one level.

But your question specifically states 'from' one platform, you can get to both, but only depart from one of them (unless the summer trains to Okehampton are running through from platform 1)
But I don't specifically state passenger train! What arrives must depart!

Zürich HB has more than three, as far as I am concerned, Stuttgart 21 is not nearly finished (2021 was the target (hence the name) but it's now delayed till 2022).
Looking at Google street view the curve platforms at Sloterdijk seem to be on the same level as the high level platforms.DB say Koln-Deutz has two platform levels , the sloping platform starts off at the upper level.

Although this doesn't count as an answer, Helsinki's central station was built with platforms on three levels. All of the mainline platforms (19 of them!) are at ground level, then the Metro uses an underground island platform which is accessible without leaving the concourse building. Finally, there is a third underground island platform, below the Metro one, which was excavated "just in case" a second Metro line was ever required. To date it has never been used, and has no tracks.

Notably, not only is Helsinki's Metro the same track gauge as the mainline network, but there is a physical connection between them. It's very rarely used (only for delivering new rolling stock, I think) but it is intact as far as I can tell.

Sorry, not Matlock. Platform 2 is leased by Peakrail and isn't part of the network rail station.

The platform I'm thinking of has no service in June and July.

Would this be Bordesley by any chance? I suppose since both platforms only get used during football season this would count.

Yes, its Bordesley near Birmingham Moor Street. The normal timetable is for one (down) train on Saturday afternoons, but several additional trains stop when Birmingham City have a home game at St Andrews. I suspect it is only the football keeping it open.

Actually to split hairs, the underground station is called Plaza Miserere. Once (de Septiembre) is the above ground train station at the same location. The Trains (not underground) had until recently service down into the underground station (albeit on separate lines, different gauges and electrification).
