I had a late train enter from Hyndland. The slot was requested before the train was normally due to enter, I granted it at first but cancelled it once I learnt about the delay to let a train through in the opposite direction first. Then I set the slot again. At 22:13:40 the train finally entered. Then at 22:14:35, the slot was requested again - at the same time, the slot indicator went out, and the train got an ACOA at signal YH519. Attached save is from just after the ACOA. Just had this happen again. It seems to be caused by having two trains in quick succession from Hyndland. If the slot for the second one is requested, the first one gets an ACOA at signal YH519.

Just managed to get a save, just 10 seconds before the ACOA happens. Also attached an earlier save (231) where I hadn't granted the slot yet but the two trains are already scheduled to enter in quick succession.
