Just read the RAIB report for the Lindridge Farm Crossing incident and what an interesting read it is.
A simple error on a diagram that could have indeed caused a serious incident had it not been reported. I have seen in my own industry the results of typographical errors and things such as incorrect tagging and what it can lead to regarding incorrect isolations and potential live electrical cables as a result.
I would recommend it as a good read and further understanding as to what goes on in the signalling field and why the real thing (as well as Sim Sig development) needs such a great deal of accuracy and checking by the checker.

NR approve all scene plans and would often pick up such errors. If the design checker and NR do miss it then testers (such as myself) would usually pick up on such an error as indeed they did in this case. The trouble was that it looked like a simple typo on existing records and the ramifications were not appreciated when the tester in charge and NR agreed to defer the test log.
