No failures or delays are enabled in current version (3.4) of KX in any difficulty, except Bad Weather. Other modes have all sliders set to 'None'.

Yeah, but aren't these presets meant to provide a sensible default that's actually playable and interesting (well - save for the Difficult setting sometimes)? Setting it to "something different" quite often yields either too much delays, too long or too short (in one MP, someone set it to several minutes, so it didn't pay off to plan anything special to deal with the failure - usually before any train got there, it was cleared).
