I had a very late 2Y14 enter at two locations at the same time - Watford Up Slow but somewhat unexpectedly at Down West London.

There's nothing wrong with that- the one on the main line is a Birmingham- London stopper, the other is Clapham- Willesden service. It just happens that both use the 2Yxx headcode series. It's not like you have two versions of the same train.
