4A40 called to request permission to depart Morris Cowley. Permission granted. Some time later I realised it didn't called for the token. Quick search for 4A40, train not in F2 train list, F4 shows train marked as having entered.

I experienced the same issue myself. This is with the 15/10/2009 timetable (04:45 start). I also had an issue with Banbury Road Sidings on the same timetable. I successfully operated the ground frame so that train 6A14 moved off the running line into the sidings, closed the ground frame and clicked on Train Arrived. 6A14 left the sim. However, the Tokenless Block apparatus continued to show Train in Section. I had to authorise the following train to PSAD to enable it to enter the branch. Train in Section continued to show until I clicked on Train Arrived Complete when this train had completed its return trip to Bicester. While I'm on the subject, I have not yet seen anything come up on the tail lamp camera. I'm not sure what this should look like but I haven't seen any change on the screen when trains are coming off the branch. I'm running Loader version 4.6.4 with no outstanding updates. Is there a procedure to follow if the tail lamp camera isn't seen? It's alluded to in the manual, and I know there are procedures on the railway which would have to be followed if it wasn't - usually stopping the train and asking the driver to check it is complete; however I assume this isn't an option in the sim.

I've found the trick is to give Train Arrived after restoring the points but BEFORE giving the release back. The animation of the camera is a small red dot moving along the upper horizontal line. Once the train has cleared the overlap past 87 signal, you can click "Train arrived complete".
