Currently, if a train sees an ACOA but isn't due to move for some time, it will only call about the ACOA when it would otherwise actually start moving. The event could conceivably have been several hours previously.

Surely it's more logical for the driver to call as soon as he observes it (after, if necessary, bringing his train to a stand)?

Is your Loader up to date (v4.6.5)? This scenario was changed a while ago; trains now ring in for an ACOA as soon as they see it if they're stopped. They also won't ring in more than 10 minutes before departure.

I have indeed seen this behaviour with v4.6.5, although it might be a more complex edge-case. Specifically, on the York sim:

- An ECS starts from York P4 and is booked to reverse in Holgate Loop, which is presently occupied by a freight. ARS calls it into P3 and then waits.

- I set the route from Y232 to Y202, ie. from P3 to the Up Leeds, where there's a shunt signal to permit reversing. Y232 clears to yellow.

- ECS calls in complaining about being held at a red. Y232 is still showing yellow. I tell him to wait 5 minutes.

- Train List shows ECS is "waiting for correct route to be set", so I cancel the route, convince the freight to move off early (it's a TT conflict), and set the route to Y622 (the loop) instead.

- Sim reports that cancelling the route caused an ACOA. ECS does nothing for now.

- At the end of 5 minutes, ECS calls me complaining about the ACOA. He's been sat staring at a call-on aspect for the past 3 minutes, and claimed he couldn't see the yellow before that...

I have this issue too.

For example - last night's Paisley host. Driver of 5W99 (self-created TT for a heavily-delayed train, which was made ECS) approaching the starter at Paisley Gilmour St from about 3 signals in advance. I put the signal on the junction back to allow a late-running train through...driver carries on at normal speed with NO report of ACOA. As soon as the late train passes through, I clear the signal again. 5W99 rolls towards it at line-speed, suddenly comes to a stop, phones up.

What?

It does sound like a bit of interplay rather than a simple ACOA. Logged in our issue tracker for further investigation.
