I purchased this from eBay recently, and considering its rarity (sorry if the person I outbid was one of you, I was very nearly not going to buy it but alcohol got the better of me!), small length, and likely usefulness to timetable writers, I'm willing to scan this for anyone who wants it. Let me know (might be best to send a private message if you want it as I'm not sure if I'll remember to look at this thread).
