I'm currently running a Saltley save and have suffered a door failure to 2N51 at Selly Oak. The issue occurred at 12:02 with an expected departure at 12:05 but it is now 14:19 and the train is still stuck with a pretty mammoth queue behind it now haha.

I've never experienced a delay to a train of this length of time so just thought I'd find out if I have encountered a bug or whether it is something that just sometimes happens as part of the fun and games?

Can you include a save file of the situation, please?

Don't judge my performance - Saltley is tougher than it looks for someone of my ability haha.

Narrowed it down a little.. it seems 2N51 got an ACOA at some point, and is trying to call to report it. When you abandon its timetable, it will actually put the call through.
I've experienced similar bugs myself where a train is in multiple states at once and just freezes. For example, given permission to leave a station through red signal when points are set wrong, the train's status will stay on "waiting for right away", until the points have been aligned.

Initially I instructed the driver that the signal would not be changed when they initially called in the door fault - a 3 minute delay didn't seem worth changing the signal for. I did then change it hours later in a failed attempt at nudging it into action and it was then that it received it's ACOA.

I will try your idea of abandoning it's TT when I go back on and report my findings!

Okay, so that was not the issue itself.
My own testing also confirms that, causing an ACOA before or after answering the delay phonecall will not get the train stuck.

And causing a ACOA didn't prompt the working into life either.
At no point after the delay being advised did the status of the train change from 'Delayed at Selly Oak' - if it was an issue with the driver trying to make a call would it not change to 'On Telephone'?

Anyway, your idea of abandoning the timetable did bring it immediately back to life. As soon as I did that I received the ACOA call from the driver and then it began to move. I ran it without timetable to Longbridge and then reinstated the TT so it could continue it's next working to the turnback siding.

Safe to say that things are interesting along that stretch of track now as 2+ hours worth of delays from New Street are flooding out :D

That's what I previously tried to explain. The "delayed" status has a higher priority than the "on telephone" status, so it is shown instead, and no call is made. The same is true for "waiting for right away" and "points set against train".
