Because the train was passing the signal at danger. The signalman did not check the catch points were set correctly before authorising the train past the signal.

Yes, trap points. Catch points are found on gradients to derail runaway wagons (see Royston sim). As you say, interlocking would have prevented derailment had the driver not been authorized to PSAD. Good Wikipedia article here. Last edited: 03/07/2013 at 00:17 by maxand Log in to reply |

I think there was an extensive topic regarding this "incident" which was circulating around about the time it happened. Possibly more information (or more accurately speculation) on there.
