The Rail Accident Investigation Branch (RAIB) has released its report into a collision between a stoneblower and ballast regulator near Arley, Warwickshire, 10 August 2012.

The RAIB has made three recommendations.

The Rail Accident Investigation Branch (RAIB) has released its report into the Partial failure of a structure inside Balcombe Tunnel, West Sussex 23 September 2011.

Report not yet available on RAIB website.

The RAIB has made five recommendations.

The Rail Accident Investigation Branch (RAIB) has released its report into a dangerous occurrence involving an engineering train at Blatchbridge Junction, near Frome, 19 March 2012. The RAIB has made seven recommendations.

Thanks to all who post details of the RAIB reports. However, it would be nice if we could have a consistent format rather than some links referring to the RAIB summary page and others directly picking up the report. For myself, going via the summary page just adds an extra click into the process when I am trying to read what happened.

The Rail Accident Investigation Branch (RAIB) has released its report into a fatal accident at Bayles and Wylies footpath crossing, Bestwood, Nottingham on 28 November 2012. The RAIB has made four recommendations.

Couldn't see any information on RAIB site as to what the revision was.
Page 2 has the Description of Change. Paragraph 13 and Appendix B.

This update includes a new addendum covering difficulties that passengers had in operating the Passenger Emergency Alarm (PEA) handles that were reported to the RAIB by a passenger after the original report had been published. Actions taken by the train operating company to address these difficulties mean that no additional recommendations have been made."
