Presumably they don't foresee anything being gained from investigating the incident. The RAIB only investigates a minority of incidents, mainly those are novel (e.g. the SPAD at Stafford, which had a number of very interesting factors) or likely to be repeated again with more serious consequences (e.g. cases where some definite change to normal operating practise is required). After a look at a couple of news articles, the Blackfriars incident doesn't look like it had much of a potential for danger or injury, and it more of an operational inconvenience.

Rail Accident Investigation Branch (RAIB) has released an update to its report into a collision at Buttington Hall level crossing, near Welshpool on 16 July 2013. The report has been revised to correct an error related to the actions of the signaller and to clarify the wording of the learning point.

Amazing that the train driver was uninjured even though the train's speed was 74.7 mph at the time of impact! Do they wear safety belts? Thanks for posting.

I see no reason he would be injured, he has several hundred tonnes of train and various laws of physics on his side.

"AHB crossings are arranged so that if a crossing is triggered spuriously and no
train is approaching it, the circuit times out after a few minutes and the barriers
are raised and the wig-wags extinguish. However, if a train triggers the crossing
but stops before reaching it, as happened here, the crossing remains in the
activated state with the barriers down and wig-wags flashing."

What exactly happens here? By "spurious" do they mean some temporary track circuit failure that disappears and then a few minutes later the AHB is released? Would a "SimSig style" track circuit failure for a long time cause the AHB to be locked?

Yes, if it's a hard failure the barriers will stay down. If the track fails and re-clears or if an animal steps on a treadle the crossing will reset.

The RAIB’s report into the accident involving a track worker being struck by a train at Saxilby, Lincolnshire on 4 December 2012 has been updated to version 2. The changes affect paragraphs 36 and 38 and comprise clarifications to the requirements in the railway rule book for the type of work that was being undertaken at Saxilby on the day of the accident.
