Rail Accident Investigation Branch (RAIB) has released its report into a derailment at Liverpool Street station, London 23 January 2013.

RAIB has made three recommendations and identified six learning points.

Rail Accident Investigation Branch (RAIB) has released its report into the unauthorised entry of a train onto a single line at Greenford.

RAIB has made three recommendations and identified two learning points.

Rail Accident Investigation alert: RAIB investigation update - Channel Tunnel

RAIB is working with the Bureau d’Enquetes sur les Accidents de Transport Terrestre (BEA-TT), the body responsible for the investigation of railway accidents in France, to jointly investigate a fire on-board a train in the Channel Tunnel.

You're right that it was very dangerous, of course, but I actually thought it was a clever move considering the area (although I don't know if it was as "inhospitable" as the report originally suggested).

Network Rail have basically said "no, not now, not ever" about air ambulance landings but you'd hope that if safety could be guaranteed (removal of current, trains stopped, etc) it might be an effective way to reach people in situations like this.

This report also contains what I believe deserves understatement of the century award...

"A railway line is a potentially hazardous place on which to land a helicopter."

The report also mentioned concerns about how safe it was structurally - track and ballast aren't exactly flat or stable surfaces, and if you drop a helicopter skid on something like a cable trough it's going to drop straight through.

I can see the issues with it (there's probably more, even, than are mentioned in the report)

I'm just interested that they've ruled absolutely that it cannot happen. where it may be beneficial (even the difference between life and death) on a case by case basis. Considering how rare it would be that an air ambulance would need to land on the railway, I just assumed in a situation where it *could* happen (on the spot risk assessment?) and *needed* to happen (vital need to get a patient to hospital), there might be exceptions.

In any case, this report obviously had a little bit extra about it because of the worker's appearance on 24 Hours; sort of made it feel more human - it wasn't just a faceless railway worker. I do wonder sometimes why they don't reveal the names of the people involved in RAIB reports - you see people named in older reports.

Having spent two years living in Reigate and overlooking the incident site, it's a fairly inaccessible industrial estate, even though East Surrey Hospital is just down the road, it's not an easy place for an ambulance to get to, especially once traffic starts to back up.

Indeed! I was at the college there in Gatton Point for a year, and spent many an hour in Watercolour, and the entire line (both of them) are really high up with sheer drops both sides. I actually thought the Quarry Line ran on a viaduct along there, as inaccessible as it seems. Still, risky move by the pilot…
