On a quick scan through I couldn't see many guesses for Q2, so I'll throw in
Hoveton and Wroxham change for Bure Valley Railway.
I reckon there must be some a fair bit longer, but 43 is a starting point.

I'm wondering what Birmingham International's full sign says - presumably with reference to the airport and NEC it must beat Wroxham.

6: I'm going to claim a bonus point: there used to be a YORK road on the Underground.

8: Madrid line R (Principe Pio to Opera). Taipei has two: Beihou to Xinbeitou and Qizhang to Xiaobitan. Hong Kong has Sunny Bay to Disneyland. I'll also point out that both Paris and Barcelona have point-to-point funiculars that are included on the metro map as metro lines.

11: The furthest out termini of the Underground were Brill and Verney Junction. The Southend service that Kbarber mentions went to Shoeburyness. The District also once ran to Windsor. But I've also been on LU trains to Rugby (via Aylesbury and Bletchley), Portsmouth Harbour, and Dungeness via Fokestone.

13: Acton Main LINE.

21: is it Wick?

22: Limerick Junction was laid out so that a through train couldn't use any of the platforms without reversing.
