Have been really looking forward to this one - I assumed it would just be negligence on part of the lookout but seems that use of this lookout equipment in areas where lookouts see the train long before they need to notify the group is a recognised issue. RAIB does employ an inspector with huge experience in human factors so no doubt he was involved with this one.

Desperately close to a serious tragedy. The CCTV still of "three seconds before" sends you cold.

I was honestly very surprised to read that it wasn't common practise to use some sort of detection such as a treadle to fire off the warnings, despite the technology being fairly simple. Admittedly it doesn't cover all possibilities (e.g. trains running wrong line), but it's surely a far more reliable system - plus you save on labour, for bonus points.

Indeed. It seems that the 1.4 equipment was only suitable for human operation and had the vigilance switch. 1.5 uses movement sensors to test vigilance and can also be operated by said treadles, axle counters etc.

I was also amused (darkly) by the paragraph about the alarming regularity with which the lookout had to confirm vigilance. The German manufacturer stated previously the interval was far greater than 20 seconds when the equipment arrived in the UK. "It was not known why this was changed"

I have never known RAIB be this active! Here's a "secret" report (i.e. wasn't reported on their "current investigations" list - EDIT: Although think they said they were going to do it on their old website). They sometimes do this for the class investigations.

I suspect it's just everything lining up coincidentally, since I've noticed especially there had been a huge drought up to this point. Probably just a load of investigations happening to complete at the same time. Unfortunately I suspect this means we'll have another drought!

Yes quite so! There's only one left which is marked as "consulting" - that's the meeting of the fallen Newbury signal and the HST! The rest are quite a way from publication.

Nothing in here to suggest there's been anything other than a serious irregularity, I'm afraid. In fact, for the first time RAIB have alluded to the fact this isn't the first time.
